Whether you are part of our old family “Taking Steps” to join your team member with Lifespan Behavioral Health Services (LSBH) or new to LSBH, we require you to fill out this form.
Your team member had your complete chart transferred when they took their first steps to LSBH to assure continuity in your care but we want to make sure we have it right so there is no delay in your treatment.
If you are brand new to our team at LSBH, we want to assure you have the best experience possible. This experience starts with building you a chart, checking your insurance eligibility, and making sure you are aware of what costs you might accrue. We do our best to get this process completed within 2 business days. Once we are sure you can schedule with us, you will receive a call from our intake coordinator Evelyn Medina (intake@lsbhtherapy.com).
